Complaints procedure form
We need you to give us certain
information so that we can follow up your complaint. As we work with
the London Borough of Waltham Forest, we may need to pass your details
on to their service areas. We will keep information that you give us on
our computer database in line with the Data Protection Act 1998. We
will only use information to deal with your complaint and to help us
improve our services.
When
you contact us to make a complaint, we will ask you to fill in and
return to us the complaints procedure form. On the form we will ask you
for your personal details such as your name, address and phone numbers.
We will also need to provide full details of your complaint, including
whether or not it is the first time that you have complained about the
issue and how you made your complaint.
The
complaints procedure form is also available in Braille, in large print
and as an audio tape. We can also provide a translated version.
Please ask us if you would like one of these formats. To make sure that
everyone is receiving fair and equal levels of service we will also ask
you to complete an equal opportunities monitoring form.
We
will make sure that it is easy for you to make a complaint. You have
the right to complain and we want to make sure that everyone is treated
fairly. If you choose not to fill in the complaints procedure form, it
does not affect the way we deal with your complaint. The information
you give us is strictly confidential and we will only use it to help us
deliver the services you want.
